Creekside Rehab and Nursing is a 41-bed skilled nursing facility located in picturesque Portland, OR. We have been providing personal skilled care since 1970. Our community provides 24-hour nursing care from our highly trained Registered Nurses and Licensed Practical Nurses who focus on the individual needs of our residents. Our entire nursing staff understands the common goal of returning our patients back to their highest level of functioning possible.
Creekside Rehab and Nursing is certified for short-term skilled Medicare stays. This allows individuals to achieve the highest level of recovery possible from a traumatic event through rehabilitation and therapy so that they may return home as active members of their community. We also provide long-term care for those who require assistance with everyday tasks and those with other medical needs. Our dedicated and compassionate employees strive to meet the various needs of each unique patient.
Services We Offer
Skilled Nursing
Our 24-hour nursing care is provided by RNs, LPNs, and CNAs in a private or shared room. We also provide pharmacy and diagnostic services along with mobile dental care and weekly physician visits.
In-house Therapy
Our in-house Speech, Occupational, and Physical Therapy staff are committed to creating individualized therapy programs that focus on your personal goals. We encourage each resident to attain their maximum potential of independence and ability.
Respite, Hospice and Palliative Care
Our respite care is designed to meet the physical and medical needs of a resident during a short-term stay. Palliative care can be provided at any time during the course of a serious illness. Hospice care focuses on quality of life and the comfort of each individual.
Social Services
Our social service team assists both residents and their families during transitional periods. We also provide essential information, manage requests and concerns, help in care planning and discharge planning for each resident.
Dietary Services
We offer adaptive meal plans and diets for our residents based on physician orders and assessments from our registered dietitian.
Activities Program
A positive and uplifting atmosphere can set the tone for a resident’s stay. We employ outgoing and friendly professional staff to create social programs and activities that we specialize to match the capabilities and needs of each resident.
